As part of your child's passport application, you will need to provide proof of identification.
You must provide a certified copy or the original of one of the below:

Any original documents submitted will be returned after the completion of your child's passport.
The birth certificate should be issued by the office of vital statistics of the state, county, or city where your child's birth occured, show the child's full name, date, and place of birth, list the full names of both parents, bear an embossed, impressed or raised seal and signature of the issuing authority, and indicate a registration or file date that is within on year of the birth. | |
A hospital-issued birth certificate is not the certified copy that is needed. | |
If the mother's name on the birth certificate does not match the parental proof of identity submitted with the child's application, an official name change document for the mother's name change must be submitted along with the other application documents. |
