| You are under 18 years old and do not have a state or government issued identification. | Your parents may provide their identification (matching your birth certificate) as proof of identification in place of your own. | |
| None of the above items are available to prove your ID. | You must submit at least 6-10 items from the combination of personal documents and public records from the Secondary Identification list. Documents that bear your photograph, signature, and/or physical description are particularly helpful (more info). |
