Information for

You must provide one of the below if you are completing a sealed document process (New Passport, Lost Passport, Damaged, Child's Passport) or if completing an unsealed process (Passport Renewal, Passport Name Change) with a processing time of 3-days or faster. 


PLANE TICKET OR TRAVEL ITINERARY

A copy of your plane ticket, airline email confirmation, or travel itinerary generated by a travel agency showing your name and date of travel.

Please note: typically the letters below are acceptable as proof of travel to the US Department of State, but it remains up to their discretion if it will be accepted or not.

BUSINESS LETTER

Business travelers may provide a business letter if neither of the above is available.

EMPLOYMENT PURPOSE TRAVEL

If you need your passport for employment purposes, a substantiating letter from the HR department of your company may also be required.

PERSONAL TRAVEL

If you do not yet have a plane ticket or travel itinerary and are unable to provide one before submission, a personal travel letter may suffice.
